Efficient care management always begins with a comprehensive, holistic assessment of the client’s present circumstances. It’s the first piece of the puzzle. After all, the assessment is what we use to construct a personalized, unique plan, tailored to fulfill and meet all of your loved one’s needs.
Our experienced Case Manager and Registered Nurse will meet with you and your loved to evaluate the prospective client’s medical, social, and psychological functioning. Physical and mental limitations as well as nutritional requirements will also be closely considered as part of determining an on-going action plan. Essentially, the initial consultation is a vital tool that identifies strengths and weaknesses and gives us a sounding board.

Management
Aging Grace Care Management supports all the daily oversight for your loved one to remain in their own home inclusive of performing the functions of their ADL’s. This includes safety reviews, bill pay, money and environmental management.
Direct Care Coordination
In providing a hands-on approach, we monitor and give oversight of care in person, and at your convenience. This can be done on a weekly, biweekly, or monthly basic. It’s up to you! Our team coordinates in-home care, medical appointments, provides transportation and serves as the conduit when discussing physician recommendations and ongoing clinical based care. AGCM will provide consistent communication, detailing updates to family members and other providers involved in our client’s care.
Navigation of Health Care Services
It is easy to become overwhelmed by our society’s vast and intricate healthcare system, specifically the array of services offered to the geriatric population. Relax! At AGCM we have the expertise to quickly and easily outline the available options and assist you to make the best decisions for your loved one (and you!).
We will direct you to the appropriate providers and help you conquer any impediments that may arise during the arrangement of care. We truly become our client’s greatest advocate- embarking on a campaign to make certain that the health care services they obtain are the right ones- both quality wise and financially.
Transitional Care Management
Mom is being discharged from the hospital after suffering a bad fall. What now? Aging Grace Care Management works to prevent readmissions in these instances when a client has completed a stay at a facility such as a hospital or skilled nursing facility. In an effort to do so, we will see to it that the client’s wellbeing is still being closely considered even after being discharged.
This includes helping to schedule/accompany any necessary follow-up visits, attaining any new prescriptions and ensuring their proper administration, discerning and implementing any patient instructions given upon discharge, and so much more. When your loved one returns to the comfort of their own home, we want them to stay there!
Placement Services
Despite Aging Grace Care Management’s very best intentions to always avoid such, unfortunately, sometimes it is not possible for a loved one to remain in their own home, whether it be for a short period of time or permanently. In these situations, we will syndicate our knowledge of facilities in the area with the information we glean from the initial assessment to establish the pathway for a seamless transition from one healthcare setting to another.
Aging Grace Care Management will be there every step of the way-whether it be accompanying you and your loved one on tours of facilities or filling out necessary paperwork. We can also even help you organize and simplify the actual physical aspects of a move.
